Health Services is open Monday through Friday.

8:30am - 12:00pm and 1:00pm - 5:00pm

Summer: 8:30 am - 12:00 pm and 1:00 pm - 4:30 pm

Walk-In Hours are from 9:30am-10:30 am and 2pm-3pm Monday through Friday.

Services During the Academic Year:

  • Physical Examinations
  • Exam for people assigned female at birth, Pap smear
  • Illness evaluation and management
  • Injury care/wound management
  • Minor procedures
  • Sexual health – birth control, STI education/testing/treatment
  • Wellness counseling
  • Onsite Rapid Testing for COVID-19, strep, mono, pregnancy, UTI, blood glucose
  • Blood draw for send out laboratory tests through Labcorp
  • Collaboration with home healthcare team for continued care of chronic conditions

Health Records and Immunizations

All full-time (first year and transfer) students are required to complete the following forms on the :

  • Medcial History Form
  • Immunization History Form - A signed/stamped copy of your immunization records must be uploaded into the portal for dates to be verified.
  • Tuberculosis Questionnaire
  • Consent to Treat form (if under the age of 18 prior to the start of the semester)

It is important that you complete all necessary forms with vaccination proof into the by January 15th for Spring semester start and by July 15th for Fall semester start. If forms are not submitted by January 15th/July 15th, residential students not in compliance with the immunization requirements may have their room keys withheld upon arrival to campus. Commuter students who are not in compliance with the immunization requirements will be restricted from registering for subsequent terms until compliance is obtained. 

Immunization Exemptions

  • For medical exemption requests: Please ask your treating medical provider to write and sign a letter (on official letterhead) indicating the nature and probable duration of the medical condition(s) and/or circumstance(s) that contraindicate a vaccination(s).
  • For religious exemption requests: Please fill out the religious exemption certificate.

All exemption requests should be submitted by July 15th for fall semester or by January 15th for spring semester. Submissions should be made as a PDF on the student health portal via "Document Upload." Please note, the portal is confidential and only Health & Wellness Center staff can view the portal. 

COVID-19 Vaccination Information

There is no COVID-19 vaccination requirement for 91ÉçÇø students, staff, and faculty. All members of our community are encouraged to continue getting the most up-to-date COVID-19 vaccine to maximize protection against the most serious effects of COVID-19 (CDC guidance).

    When the Health Center is Closed

    In the event of an urgent or emergency illness or injury when the Health and Wellness Center is closed, on-campus students can call the Department of Public Safety at 847-735-5555. An officer will respond to all calls, evaluate your condition, and arrange for transport to the nearest emergency facility if needed. If it is a life threatening emergency call 911 to be transported by ambulance to the nearest emergency room. In the event of an emergency off-campus, students should call 911. 

    If your issue is less urgent but can’t wait until the Health and Wellness Center is open, there is a local clinic that has hours of operation in the evenings and on weekends:

    What warrants visiting the hospital?

    • Emergency Care for illness or injury that can’t wait until the clinic opens.
    • Services not provided at the Health and Wellness Center such as radiology (x-rays, ultrasound, etc).
    • Inpatient care.

Please call 911 or go to your nearest emergency room if you are experiencing:

    • Choking or gagging
    • Severe Hypertension or Hemorrhoids
    • Head injury
    • Possible broken bones
    • Cancer
    • Patients who want to hurt themselves (suicidality)
    • Patients who want to hurt others (homicidal)
    • Schizophrenia or hallucinations

    What is a Nurse Practitioner?

    A Nurse Practitioner (NP) is an advanced practice nurse with graduate level training who provides healthcare services similar to those traditionally performed by a doctor. NP’s diagnose and treat health problems and focus on health promotion, disease prevention, health education and counseling. They help patients make wise health and lifestyle choices. 

    What is a Registered Nurse?

    A Registered Nurse (RN) is a licensed medical professional who provides hands-on medical care. RNs provide basic care, health promotion, disease prevention, health education and counseling.
